In more acute situations, such as during a hospital stay, our Care Managers will communicate personally with physicians, physical therapists, and other medical professionals regarding your loved one’s treatment. After developing your personalized care plan, we will choose the most qualified caregiver with the required knowledge, skills, and abilities while emphasizing a fit in personality and cultural competency. Our Care Manager can help work with the transition from hospital to home by working in tandem with a multi-disciplinary team of healthcare professionals assigned to the patient.
