How the Agency Switch Works
1. The Client Must Voluntarily Choose to Switch
Under Pennsylvania law, clients have the legal right to select or change their home care agency at any time. The decision to move to Chosen Family belongs entirely to your client.
2. Verify Medicaid & Health Plan Eligibility
We quickly verify that your client is Medicaid-waiver eligible or enrolled with a participating Community HealthChoices (CHC) Managed Care Organization (MCO).
3. We Handle All the Paperwork
Our team manages the transfer process from start to finish. We handle the administrative steps with the health plan so you don't have to stress about administrative roadblocks.

*If a caregiver works a regular schedule of more than 40 hours per week, their pay rate will be averaged out with a lower regular rate of pay plus an overtime rate (1.5 times regular pay rate earnings) that equal an average of $15 per hour.
