Our Care Managers begin by conducting either a free Home Care Assessment or Care Management Phone Consultation. Home Care Assessments can be provided in homes, hospitals, skilled nursing facilities, assisted living communities, etc. After the assessment, your dedicated Care Manager will develop a customized Home Care and/or Care Management plan. The family’s involvement and approval is encouraged.
Care Managers provide expert guidance by helping clients and their families to anticipate and manage the medical, financial, insurance, entitlement, legal, personal, social, and emotional aspects associated with aging, disabilities, medical conditions, recovery, chronic illness, or crisis. Professional care management is available to individuals, families, guardians, attorneys, trust officers, wealth managers, physicians, health care providers, social workers, discharge planners, and senior living communities. We value client confidentiality.
Comprehensive, Personalized Assessments
We offer complementary care management consultations, conducted by a Care Manager. Our care assessments begin by gaining a complete understanding of the individual’s medical history, physical abilities and needs, nutritional requirements, cognitive functioning, social needs, legal standing, and financial situation. Our Care Managers will then develop a customized care plan.
